Immunization Requirements

Ohio law requires students to meet state immunization requirements before attending school. Immunization records should be submitted on or before a student's first day. Students who are not up to date on required immunizations and are not actively completing them may be excluded from school after 14 days, in accordance with state law.

Student Health Information

  • School nurses maintain student health records, including immunization documentation, medical information, and other health-related records required to support students safely during the school day.

Families should notify the school nurse whenever there is a significant change in a student’s health, medication, or medical needs.

  • Students must meet Ohio’s immunization requirements for school attendance. Families are encouraged to provide updated immunization records as early as possible and contact their school nurse with questions.

  • Our nursing staff works with families and healthcare providers to support students with individual medical needs during the school day. Depending on a student’s needs, this may include medication administration, diabetes care, allergy management, seizure plans, or other physician-directed health services.

Families should contact their child’s school nurse to discuss individual health needs and required documentation.

  • School clinics provide care for illness, injury, medication needs, and other health concerns that arise during the school day.

Families are encouraged to keep emergency contact information current and communicate directly with the school nurse regarding significant medical concerns.

  • Knowing when to keep a child home helps protect the health of the entire school community.

  • Beavercreek City Schools provides health screenings in accordance with state guidelines. Screenings may include:

    • Vision
    • Hearing
    • Preschool or kindergarten screenings
    • Additional screenings as needed

Families will be contacted if follow-up with a healthcare provider is recommended.

  • School nurses also support health education and wellness initiatives throughout the district. Programs may vary by building and grade level.

  • Ohio Revised Code section 3701.12 and 3313.671 calls for students entering the 7th grade to obtain a tetanus, diphtheria, and acellular pertussis vaccine (Tdap) and Meningococcal (MCV4) vaccine. Therefore, your current sixth grader will need to show proof of having received these vaccines before they return to school in August. Please bring proof of vaccination to your middle school nurse.

Immunizations are also available at the Montgomery County Health Department at minimal or no cost. You may call them at (937)225-5700 for the office hours. Students must be accompanied by a parent or legal guardian.

If your 6th grader gets immunizations while at a doctor's appointment please provide documentation to the school clinic.  Many doctors administer the Tdap and Meningitis immunizations during routine check-ups.
